Patient Rights

Patients have a Bill of Rights that is posted in our lobby. Patients who refuse to follow medical advise or exhibit unacceptable behavior may be dismissed from our practice. All patients are required to provide picture identification.

Payment Terms

We are happy to process your insurance forms for you. Healthcare insurance is intended to cover some but not all, of the cost of your medical care. Most plans include co-insurance, a deductible, and /or other expenses that must be paid by the patient. If you have insurance, please bring your plan information with you each time to assure that you receive the maximum benefits to which you are entitled.

Payment is expected at time of service. We do offer a discount for private pay individuals if paid in full at time of service.
